AT A GLANCE

The United States Border Security Act, signed by President Bush in May, requires airlines and cruise lines to collect the following information from their passengers, effective January 1,2003.

 

All passengers sailing to or from the United States must provide the following personal information prior to departure:

 

Complete Name

Date of Birth

Gender

Residence Address

Citizenship

 

In addition, non-U.S. citizens must provide:

 

Passport Number

Country of Passport Issuance                    

Passport Issue Date

Passport Expiry Date                                

Country of Residence

Alien Registration Number (if resident of the United States)

U.S. Address While Visiting U.S. (if not a resident of the United States)

 

Passengers who are citizens of a non-visa waiver country (those not listed below), and are not US or Canadian resident aliens, must also secure a valid U.S. visa and provide the following:

 

U.S. Visa Number

Visa Issue Date

Place of Visa Issuance

 

Visa Waiver Countries (no U.S. visa required from the following):
